Therapy notes keyboard for iPhone: NoteKey expands SOAP, DAP, BIRP and GIRP note templates with fill-in fields in any app, including your EHR.
What it does
- Type .dap and a whole progress note appears with fill-in fields you tap through, while a progress chip shows where you are in the note.
- 15 note templates, including SOAP, DAP, BIRP, GIRP, intake summary, mental status exam, risk assessment, safety plan review, telehealth, group, couples and family, treatment plan review and discharge. Three are included without a subscription, and Pro adds all 15.
- A library of 69 clinical sentence stems tagged by modality (CBT, DBT, ACT, MI, EMDR, person-centered, psychodynamic, family and substance use) that you can filter and search. Inserting phrases is part of Pro.
- Autocorrect built on a psychiatric lexicon from open National Library of Medicine data, which protects medication names and instruments such as PHQ-9 and GAD-7.
- Dictation with Apple's on-device speech recognition (Pro): audio is never stored or transmitted, and NoteKey formats what you said into note shape.
- A session timer and a words-typed count in the keyboard's own header (Pro).
- Four specialty packs, CBT, EMDR, family and substance use, each adding terms, templates and phrases. One pack is included without a subscription.

Is there a keyboard for writing therapy notes on iPhone?
Yes. NoteKey is an iOS keyboard for therapists, psychologists, LCSWs, LPCs and LMFTs. It expands note templates, offers clinical phrases and works in the apps you already write in, including your EHR, Notes, email and telehealth platforms.
Does NoteKey record sessions or use an AI scribe?
No. NoteKey does not record sessions and has no AI scribe. The keyboard contains no networking code, there are no accounts, analytics or ads, and nothing you type leaves your phone.
What is the difference between SOAP, DAP, BIRP and GIRP notes?
They are progress note formats. SOAP is Subjective, Objective, Assessment, Plan. DAP is Data, Assessment, Plan. BIRP is Behavior, Intervention, Response, Plan. GIRP is Goal, Intervention, Response, Plan. NoteKey has a template for each.
